Skilled Nursing
Skilled Nurses will conduct initial and ongoing comprehensive assessment of the patient’s needs. He or she will also provide counseling and education to the patient and family/caregiver regarding the disease process, self-care techniques, and prevention strategies upon returning home from hospitalization or upon being released from institutional care.
Skilled Nurses will coordinate care with physicians and the clinical staff while providing services such as:
- Initial and ongoing comprehensive assessment of the patient’s needs
- Counseling and educating the patient and family/caregiver regarding the disease process, self-care techniques, and prevention strategies
- Coordination of care with physicians and the clinical staff
- Diabetic management and education
- Management of acute and chronic illnesses
- Ostomy Care
- Wound/Burn Care
- Pain Management
- Medications management and teaching
